Which signs mean something is wrong?

Redness that spreads, swelling, discharge with a smell, the wound opening, fever, or an animal that stops drinking or eating. Any one of them is reason to call the vet the same day.

Can they take the stitches out?

That's done by the vet who operated, at the check-up that corresponds. Here the wound is cared for and watched until that day.

And the cone, does it have to stay on?

Yes, for as long as the vet says, at night too. Most wounds that open do so because somebody took it off «for a bit».
