Basic dental hygiene
Basic dental hygiene is maintenance: surface cleaning, a look at the gums and a heads-up if something needs a vet. It doesn't replace a cleaning under anaesthesia once tartar has set in, and whoever does it properly tells you so instead of selling you what it isn't.

Does it replace the vet's cleaning?
No. This is surface maintenance. Once the tartar is under the gum or a tooth is loose, a cleaning under anaesthesia is needed, and only a vet does that.
How often is it worth doing?
As a backup to brushing at home, every one or two months. What really makes the difference is daily brushing; this goes alongside it, it doesn't replace it.
Does it hurt?
It shouldn't. If the animal is in pain, has inflamed gums or a loose tooth, the session stops and they're referred: pressing on a mouth that hurts is the fastest way to make them never allow it again.
