The preventive check-up exists because plenty of things show up before the animal complains: weight, teeth, skin, joints, blood work if it's due. One check a year in adults, and two in older animals, changes the outlook for whatever turns up entirely.

How often?
Once a year in healthy adults and twice in animals over seven or eight. In puppies, the schedule is set by the vaccinations and the growth checks.
Does it include blood work?
It depends on the age and on what's found. In a healthy young adult it may not be needed; from senior onwards, the blood work is the part that gives the most information, because kidneys and liver give warning late.
Is it worth it if my animal is perfectly fine?
That's exactly when it's worth it. A check-up on an animal with symptoms is already too late for the preventive part; the value is in finding what isn't showing yet.
