How long does it take?

You start to notice it in two or three weeks practising daily. What stretches it out most is walking normally the rest of the time: if you let them pull one day, you confirm that pulling works.

Does it work for an adult dog that's pulled for years?

Yes, it takes longer but it works. What has to change is what the dog expects from the walk, and that doesn't depend on age but on repetition.
